The PHI Institute Approach

The PHI Institute uses consensus-based measurement principles to address the thorny issues associated with achieving conflict-free, science-based program assessments for disease management, care coordination and other health management programs being implemented across the US health care industry today. The PHII Board and expert panels possess the academic, scientific, business and evaluation qualifications required to assure the market place that the findings of the Institute meet the highest standards of credibility and reliability within the current state of the art and practice of population health management and outcomes evaluation.

Definition of Defined Population Health Management

Defined Population Health Management (DPHM) programs refer to any planned health intervention focused on a targeted, well-defined population where the goal is a measurable improvement in outcome metrics in health or functional status (including clinical care), economic results (including productivity and reduced health care expenditures), and/or provider and beneficiary satisfaction within the specific targeted population.

Such programs can include care coordination, care or case management, disease management, health promotion and/or disease prevention, or any other intervention (e.g. comparing different pharmacological programs, pay-for-performance programs, etc.) implemented for the benefit of a specific at-risk / high risk population.

DPHM programs can be provided at traditional medical clinics and hospital locations, employer sites, homes, community and institutional settings and can be delivered by health care providers, Disease Management Organizations (DMOs), medical call centers, remote home monitoring systems or other forms of communications, information technology, or wide-spread infrastructure improvement.
